Dear Patients,

I am writing to inform you about an important change coming into effect on April 1, 2026, as part of the new FHO+ (Family Health Organization Plus) model.

Under this updated model, cancer screening due dates (such as mammograms, Pap tests, FIT/colon cancer screening, and other routine screening programs) will no longer be tracked or maintained by our office staff. Instead, the provincial system will take a more active role in identifying when patients are due and notifying them directly.

What this means for you:

• You will receive reminders from provincial screening programs when you are due for cancer screening.
• Keeping track of your screening appointments will now be primarily your responsibility as a patient, with support from government-run reminder systems.
• Our clinical team will continue to provide medical advice, answer questions, and order tests when needed, but we may not routinely contact you about upcoming or overdue screening dates.

We strongly encourage all patients to:

• Pay attention to letters, emails, or messages from provincial screening programs.
• Book recommended screening tests promptly when notified.
• Contact our office if you are unsure whether you are due, have risk factors, or would like to discuss screening options.
